After DHMS has received your registration, you will receive a class request registration form. You will need to mail, drop off at the office, fax (435-674-6477), or scan and email to gro.21khsaw@gnik.rebma the following:

• birth certificate
• immunization records (Utah compliant)
• transcript or report card
• proof of residency (example proof: utility bill or lease agreement with current date linked to property)
• completed class request registration form

Counselor appointments will be scheduled IF NEEDED after the enrollment process is complete.

Honor class applications are available in the counseling office.

Ninth-grade honor classes are offered in:

• AP Human Geography
• Geography
• Language Arts
• Math
• Science

Eighth-grade honor classes are offered in:

• Language Arts
• Math
• Science

** Eligibility for Honors Classes:

• 3.75 overall GPA
• Straight As in the subject area
• Assessment data
• Application turned in
